Botox Not Working as Well? When to Consider a Brow Lift

If you are using BOTOX® Cosmetic to reduce forehead lines but you are still not satisfied with the appearance of the brow and forehead, consider an evaluation for other treatment options. At Beach Cities Plastic Surgery in Torrance, David A. Bray, Jr., MD, a triple board-certified plastic and facial plastic surgeon, evaluates the brow, forehead, and eyelids together when deciding whether continued injections or a brow and forehead lift is the better fit.

What BOTOX Cosmetic Can and Cannot Change

BOTOX Cosmetic reduces activity in selected facial muscles, which can soften dynamic wrinkles such as forehead lines, crow’s feet, and the vertical “11s” between the eyebrows. It can also play a role in nonsurgical brow treatment for some patients.

What it cannot do is remove excess skin or reposition tissues that have descended with age.

Signs Brow Position May Be the Bigger Issue

A lower eyebrow can create heaviness near the outer upper eyelid and change the balance of the upper face. During an evaluation, Dr. Bray looks at the eyebrow position in relation to the eyes and checks whether the forehead muscles are working to hold the brows up.

He also examines the upper eyelids because brow descent and excess eyelid skin can occur at the same time. In some cases, eyelid surgery is the more appropriate treatment. In others, lifting the brow or combining the brow and upper eyelid surgery may better match the anatomy involved.

When Surgery Becomes Worth Considering

A brow lift treats the position of the eyebrows and forehead tissues. Dr. Bray uses several approaches, including lateral, pretrichial, endoscopic, mid-forehead, and direct techniques. The choice depends on factors such as where the descent is occurring, how much correction is needed, the position of the hairline, and the surrounding facial anatomy.
